Welcome to the LFS Lab Personnel FAQ Page. We've compiled answers to the most common questions our helpdesk has received. Please look through this article before contacting our helpdesk.

FAQ Questions:

​How do I create a new account? 

You will need to complete the registration process available online using the steps below.

  1. Go to the LFS Licensing Website (http://calablicenses.cdph.ca.gov/personnel).
  2. Click on 1. Create an account.
  3. Click on Laboratory Field Services.
  4. Complete the registration form paying special attention to the password requirements.
  5. Upon registration completion, your User ID is displayed on the screen and an email with the User ID is sent to your registered email.
  6. Return to the website above and click on 2. Log into your account to access the application processing site.

​I didn't receive/can't remember my user ID. 

You can retrieve this information through an automated email.  Please note that if you just registered for an account and did not receive an email with the User ID, check your 'Spam' or 'Junk' folders.

  1. Go to the LFS Licensing Website (http://calablicenses.cdph.ca.gov/personnel).
  2. Click on 1. Create an account.
  3. Hover over Manage Account tab toward the top of the screen.
  4. Click on Recover Account.
  5. Enter the email address you used during the registration process and click Submit.
  6. You should receive an email within a few minutes containing your user id.

I can't get into the user ID management system.

We may be currently experiencing a technical issue with the user ID management system. Please notify us at LFSHelpDesk@cdph.ca.gov and we will contact you once the service has been restored.

I lost my password/How do I reset my password?​

Please try resetting your password by following the steps below.

  1. Go to the ID Management tools (https://accountportal.cdph.ca.gov/).
  2. Hover over the Manage Account tab toward the top of the screen.
  3. Click on Password Reset.
  4. Enter your user name, click Next.
  5. Answer the three security questions, click Next. If you receive an error to the questions or cannot remember them please contact us at LFSHelpDesk@cdph.ca.gov.
  6. Create a new password using the following guideline below then click Submit.
    • Must be one that has never been used before.
    • Cannot contain parts of the account name, first or last name.
    • Must be 8 or more characters in length.
    • Must include three (3) from the following four groups: upper case, lower case, number, special character (!@#$% etc).
  7. Navigate to http://calablicenses.cdph.ca.gov/personnel, then click on "2. Log into your account" to log in using your new password.

How do I update my address or other profile information?

To change your profile information (First Name, Middle Name, Address, City, Phone, Email, etc):

  1. Go to the LFS Licensing Website (http://calablicenses.cdph.ca.gov/personnel).
  2. Select option 1.  Create an online account.
  3. Hover over Manage Account and select Update Account.
  4. Log in with your user name and password.
  5. Change any of the available attributes, First name, Middle name, Address, phone, etc.
  6. click Submit to save the changes.
  7. If you have already submitted an application, Email your application ID (LFS-####) to LFSHelpDesk@cdph.ca.gov and the updated information for us to complete the rest of the process.

Helpful hints:

  • Look for any red "*" asterisks on the left hand-side of all the fields, as this indicates a field with invalid or missing information.
  • Click on the blue "?" question mark to get additional details about restrictions for this field.
  • Address Line 1 and 2 may not contain punctuation marks such as "#", ".", or comma ",".

How do I change the last name on my application?

To change your last name, submit an email to LFSRecep@cdph.ca.gov and we will request supporting documentation for the name change.

How can I check the status of the application? 

To see the status of your application, or view the documents associated with it:

  1. Log into the Application System (https://healthappportal.cdph.ca.gov/osphld/perl).
  2. Click on Submitted on the left menu of quick links.  If the menu is not visible either zoom out or click on the light blue box with three white lines at the top left of the window.
  3. Click on the application ID (LFS-####) to see additional details regarding the application.

I can't seem to pay for my application.

By design, the system will redirect you to our payment processor's screen. During this time you will not be able to access the "Pay Now" button in your application.

***The system will place a temporary 15 minute lock on your application after the pay button is launched.  A 'Return in 15 minutes to select "Pay Now" button' message will display on the payment portion of the application during this time. To see the countdown minutes, please refresh your browser by clicking on the refresh icon within browser window.  If you do not successfully make the payment, please wait up to 15 to 20 minutes before trying again.***

Possible payment problems include:

1. The payment window doesn't appear.

The payment window should appear in a new window or tab top of your current window, but sometimes the payment screen might appear UNDERNEATH all other screens on your computer. To locate the payment screen, minimize all other windows on your computer. On some browsers the payment window might be in a new tab.

Pop-up blockers will prevent your browser from opening a new window. Before clicking the "Pay Now" button, please make sure you have disabled the pop-up blocker on your browser. Mouse your cursor over "Click HERE for more information' and double-click "HERE" to open up a window for instructions to disable pop-ups.

How To Disable Pop-Ups

Determine which browser you're using then select the appropriate link for guidance. Below are links to an internet search for specific browsers you can choose from:

2. The "Pay Now" button is missing.

By design, the system places a temporary 15 minutes lock on your application when the "Pay Now" button is selected. If you are unsuccessful in your first attempt to pay, you will need to wait 15 minutes for the "Pay Now" button to re-appear.

3. I entered my payment information but it won't go through.

We only accept Visa and MasterCard. If you receive an error message or an email stating, "SERV NOT ALLOWED", this indicates an invalid type of credit card was used. 

I can't seem to get an account created. 

Look for any red "*" asterisks on the left hand-side of all the fields, as this indicates a field with invalid or missing information. Click on the blue question mark “?” to get additional instructions for this field.

Address Line 1: May not contain punctuation marks such as "#", ".", or comma ",".

Passwords: Please remember valid passwords MUST be 8 or more characters, may not contain the user's account name or parts of the user's full name that exceed two consecutive characters. It MUST contain 3 of 4 of the following; Upper Case, Lower Case, Numbers, Special Characters (!@#$%, etc). You cannot use your previous 24 passwords.

I need to ​e-mail an additional document for my submitted application 

To email additional documentation to your application, simply send an email to: lfsnewapplications@cdph.ca.gov and ensure your application number, LFS-xxxx, is in the subject line (where "xxxx" is equal to your unique application number).

Example:

To: lfsnewapplications@cdph.ca.gov
Subject: LFS-0000
Attachments: scan01.jpg
Body of Email: Attached is an updated copy of my certification ACME industries.

I have questions regarding a license renewal. 

Please go to the LFS Renewal Website (http://archive.cdph.ca.gov/programs/lfs/Pages/ContinuingEducation.aspx)

​The website doesn't seem to be working.

​At times your browser doesn't operate as it should. When a site is updated, the files saved in the browser cache may conflict with what's actually coded into the website.  Clearing the browser cache will force your browser to reload many page elements that are old and outdated.

In most computer-based web browsers, to open menus used to clear your cache, cookies, and history, press Ctrl-Shift-Delete (Windows) or Command-Shift-Delete (Mac).

  • On Internet Explorer, accept the defaults: Checkmarks in Temporary Internet files and website files, Cookies and website data, and History. Click the Delete button.
  • On Mozilla FireFox, set the Time range to clear to Everything.  Under details, ensure, Browsing & Download History, Form & Search History, Cookies, Cache and Active Logins are all checked. Click the Clear Now button.
  • On Google Chrome, accept the defaults: Checkmarks in Browsing history, Download history, Cookies and other site and plugin data, and Cached images and files. Click the Clear browsing data button.
